Welcome to the official application guide for the First American Bank (FAB) Amex Cash Rewards Credit Card. This is a fairly standard credit offering and most likely will only be of interest to pre-existing FAB customers. We recommend that all applicants review the EFS online privacy policy before submitting any personal information into their application form.
Requirements
In order to apply all applicants must:
- Be at least eighteen (18) years of age
- Have a valid US SSN
- Have a valid US street address
How to Apply
In order to obtain access to the secure application form you must first navigate to the Elan Financial Services (EFS) summary page as they are the entity charged with handling the credit end of FAB’s operations. Once you’ve successfully navigated to the page select apply now and EFS will redirect you to the application.
Step 1- Personal Information
To begin you will need to submit the following pieces of personal information:
- First Name
- Last Name
- Date of Birth
- SSN
- Primary Phone Number
- Mobile Phone Number
- Email Address
Step 2-Housing Information
You will now be expected to submit the following data regarding your housing situation:
- Street Address
- Apt #(if applicable)
- City
- State
- Zip Code
- Time at Address
- Housing Status
- Monthly Payment
Step 3-Financial Information
You must now submit the following financial data in order to be properly evaluated by FAB:
- Employment Status
- Annual Income
- Whether or not you have another source of income
- Other income Amount (if applicable)
- Other income Source (if applicable)
Step 4- Other Accounts
Indicate which accounts, if any, you currently hold with FAB.
Step 5- Additional Cards
Authorized users may be added at this time, should you want to do so.
Step 6-Terms and Conditions
Finally, take a minute to review the terms and conditions and, if you assent, select submit to submit the application.
