How to Apply for the Five Star Bank Visa Business Bonus Rewards/Rewards PLUS Card

If you are looking for a Five Star Bank business credit card with a reward points program, you will find two options; the Five Star Bank Visa Business Bonus Rewards Card and the Five Star Bank Visa Business Bonus Rewards PLUS Card. Both cards offer the same APR of 12.24% to 23.24% (APR is subject to change) and have a standard rewards program of 1 point per $1 spent. The main difference between the two is that where the Bonus Rewards Card gives you a 25% bonus on all your points earned in each month, the Bonus Rewards PLUS Card gives you a 50% bonus. Along with the extra bonus points, however, the Bonus Rewards PLUS Card does carry a $50 annual fee, whereas the standard Bonus Rewards Card has none. It’s worth mentioning, that as a signup incentive, both cards give you a cash back bonus after your first purchase, $25 cash back for the Bonus Rewards Card (and $50 cash back for the Bonus Rewards PLUS Card). How to Apply

Step 2- The application requires that you provide a large amount of information regarding you and your business. Begin by providing all of the following required information:

• Business name to appear on cards
• Name of business
• Business tax ID
• Street address
• City
• State
• ZIP code
• Years at address
• Year business started
• Business phone number
• Gross annual sales
• Organization type
• Nature of business
• Type of industry
• Country of origin
• Number of countries with offices

Step 3- Indicate whether you’d like to have cash advances and convenience checks using the pulldown menu.

Step 4- If you are already an customer of Five Star Bank, indicate which products and/or services you subscribe to by selecting Yes.

Step 5- As the business owner you will have to provide the following personal information in the indicated fields:

• Business owner type
• First name
• Last name
• SSN
• Date of birth
• Phone number
• Home street address
• City
• State
• ZIP code
• Annual income

Step 6- If you would like to transfer another balance to this account select Yes.

Step 7- Indicate Yes or No whether you would like to Add Employee Cards.

In the case that you would chose Yes, you will have to provide the following information regarding each additional employee:

• First name
• Last name
• SSN
• Date of birth
  

Step 8- Review the Terms and Conditions for the card you are applying for, either the Five Star Bank Visa Business Bonus Rewards or the Five Star Bank Visa Business Bonus Rewards PLUS. If you agree to the terms and conditions presented, tick the box (like the one seen in the example below) to indicate that this is the case.
