Requirements

To be eligible for the MidFirst Bank ASU Rewards Credit Card you must fulfill the following requirements:

How to Apply

Step 1- To start your application for the MidFirst Bank ASU Rewards Credit Card go to the page pictured below by clicking on the Apply Now link that we have provided for you at the top of this page.

Step 3- Before you start filling your application, read the E-Sign and Credit Card Application Disclosures to go over the terms and conditions of your card as well as the agreement you are making with MidFirst Bank regarding electronic communications pertinent to your account. If you agree with the terms of this document, tick the box indicating that this is the case.

Step 4- Next, you will need to indicate whether you have any existing accounts, products or loans with MidFirst Bank.

Step 5- In the next fields, fill in the following information:

Middle initial
Last name
Date of Birth
SSN

Step 6- You will then need to provide the following contact information in the appropriate spaces:

Contact phone number

Step 7- Select which image you would like to be on your credit card, either the ASU Pitchfork or ASU Sparky from the Type of Credit Card pulldown menu, then indicate your Type of Collateral Offered and Purpose of Loan.

Step 8- Enter your Promo code in the field indicated if you have one or leave it blank if you don’t.

Click Continue.

Step 9- Begin to fill out the next page by providing the following additional personal information:

Mother’s maiden name
Marital status

Step 10- Complete all your important contact information in the indicated fields. Your primary contact info is what they will use to contact you in response to your application:

Step 11- To complete the Employment & Income Information section of your application, provide your Employment Status, the details of your employment situation and all sources of income.

Click Continue.

Step 12- Enter any additional information or comments that you would like to add to your application in the large blank space, then select whether or not you will use Automatic Payments.

Click Continue.

Step 13- Look over all the information that you have completed to make sure that it is all correct.

Click Submit Application.

Step 14- Your application for the MidFirst Bank ASU Rewards Credit Card has been submitted for approval. Save your application confirmation number displayed in red in case you need to reference it later.
