Requirements

If you’d like to apply for the MidFirst Bank OSU Platinum Credit Card you must fulfill the following requirements:

How to Apply

Step 2- You should be redirected to the application page pictured below.

Step 3- To successfully complete this application you will need to read and agree to the E-Sign and Credit Card Application Disclosures, then tick the box indicating that you have done so.

Step 4- Indicate Yes or No whether or not you have any existing accounts or products with MidFirst Bank.

Step 5- To fill your application, you will need to provide multiple forms of identification beginning with the following information:

• First name
• Middle initial
• Last name
• Date of Birth
• SSN

Step 6- MidFirst Bank will need to contact you regarding your application and credit card, provide the following contact info:

• Preferred contact method
• Email address
• Residential ZIP code
• Best time to contact
• Contact phone number

Step 7- Beside Type of Credit Card select OSU Pistol Pete from the pull down menu, then select your Purpose of Loan, and ignore the Type of Collateral Offered as this does not apply to your card.

Step 8- If you have a promo code enter it in the indicated field, otherwise leave it blank.

Click Continue.

Step 9-  Select whether you are adding a co-applicant to your application then enter the following additional identification:

• Photo ID
• ID issue date
• ID issue state
• ID number
• ID expiration date
• Mother’s maiden name
• Marital status

Step 10- You will then need to provide some more contact information and housing info. Please enter your primary address when filling out the following information:

• Physical address
• ZIP code
• City
• State
• Time at address
• Property status
• Monthly housing payment
• Mailing address
• Contact phone number
• Work phone number
• Mobile phone number

Step 11- It is required that you select your Employment Status and provide whatever relevant information is demanded.  If you have any additional income, enter it as indicated and is necessary.

Click Continue.

Step 12- If there is any information or comments that you would like to add to your application, write them in the blank field seen in the example below. Then select whether you would like to use automatic payments and click Continue.

Step 13- Your application is ready to be submitted, but before you continue, review the terms and conditions of the MidFirst Bank OSU Platinum Credit Card and look over all the information you have entered to make sure that you haven’t made any mistakes. When you are ready, click Submit Application.
