Step 2-Housing Information

Please inform EFS of your housing circumstances at this juncture by supplying the following information:

• Street Address
• Apt #(if applicable)
• City
• State
• Zip Code
• Time at Address
• Housing Status
• Monthly Payment

Step 3-Financial Information

You must now submit the following financial data:

• Employment Status
• Annual Income
• Whether or not you have another source of income
• Other income Amount (if applicable)
• Other income Source (if applicable)

Step 4- Other Accounts

Please indicate which accounts you hold with FAB, if any.

Step 5- Additional Cards

Add any authorized users you would like to at this point.

Step 6-Terms and Conditions

Please read over the terms and conditions and, if you agree fully, select submit to submit the application.
