How to Apply

Navigate to the optimum rewards card summary page and click on the Apply Now link on the right hand side of your screen.

On the following page, you will need to click on the Open Now button to proceed to the application forms.

Previous address

Step 3-Employment information

You will need to choose your employment status and fill in the related fields as well as indicate the amount of your primary annual income.
