Step 6 – At this juncture, you will need to enter all of the employment and financial information listed below into the Employment Information window.

Step 7 – If you have been at your current address for less than 2 years, you will need enter the following details of your last address in the Previous Address window:

Street address
City
State
ZIP code
Length of time at previous address

Step 8 – Next, indicate your housing situation by selecting the appropriate option in the Housing Information window. Then, enter your monthly rent or mortgage expenses in the Monthly Payment field, and click Next.

Step 9 – In this step, you may choose to add other authorized users, and/or perform balance transfers (up to four). If you would like to add additional authorized users to this account, enter their information in the Authorized User window. If you’d like to perform a balance transfer, click Add Balance Transfer and provide the necessary information. Otherwise, skip ahead to the next step.

Step 10 – If a bank employee referred you to this card, enter their information in the Bank Contact window. Type any additional comments that you may like to have considered along with the rest of your application into the Comments window. Then, enter your Branch Number and CSRID in the spaces indicated, and click Next.

Step 11 – In this last step, you must read and agree to the terms and conditions for the Capitol Public Radio Endowment Affinity Card. Then, indicate that you meet the minimum technological requirements for this card (assuming that you do), and click Submit.
